What should I include in the clinic base fee?
Use the fee on your clinic’s written quote. Ask whether retrieval, fertilization, embryo culture, transfer, and storage are bundled or billed separately, then add any unbundled amounts to the closest category above.
How should I enter insurance coverage?
Enter the total dollar benefit you expect to apply across the entire plan. If your policy uses coinsurance, deductibles, service caps, or medication only benefits, calculate the estimated dollar amount with your insurer first.
What costs may still be missing?
